Getting Started

Thank you for trusting us with your therapy care needs. We will do our utmost to uphold that trust and provide you with the highest quality of care and service.  Getting started with care is easy. First, we will need a referral/order from a physician or medical care provider.  If you are using insurance, we verify your benefits and any out-of-pocket costs and then we assign you to a therapist in your area with expertise on your needs.

1. Referral/Admissions

Order/Referral For Therapy

If your therapy will be billed to your insurance, Medicare and most major insurance carriers require a physician or medical provider to oversee your therapy plan of care, so we will need a therapy order that you have or that it sent to our office.

 

Your doctor can fax an order to us directly to 866-718-5759, you can download the referral form we have linked below to be completed by your physician, or simply call us and our Admissions Team can request an order from your physician on your behalf. 

Insurance Verification

If you are using insurance, we will verify coverage and any anticipated out of pocket expenses with you before your first appointment is scheduled.  Private pay options are also available if you prefer this to using your insurance or if you do not have insurance coverage.  Many of our patients have no out of pocket costs for care.

Once all information has been verified, we will match you with a therapist in your area with expertise in caring for your condition.

2. Your First Appointment

How Will My Evaluation Be Scheduled?

You may have one or more therapy disciplines recommended for your care. Each therapist will call you to schedule your initial therapy evaluation at a time and date that is convenient for you.

Where Will My Appointment Be?

Where you want to be seen!  We welcome you at our clinic in Tamarac but the majority of our patients prefer home-based care so your therapist (and the therapy equipment) will come to you.   You can also request a community setting where you or your loved one may spend their days such as a daycare center.

Can My Family or Caregiver Be Present?

Absolutely.  We encourage family and caregivers involved in your care to participate in the appointment if you wish.

How Long Will The Appointment Last?

For most patients, the initial therapy evaluation will last between 1 to 1 1/2 hours.

When Will I Know My Therapy Plan?

 

During the initial evaluation, your therapist will review your history with you, learn more about your concerns and goals for therapy and conduct a series of assessments to evaluate your condition and to create a customized treatment plan. The plan will be discussed with you before the end of the visit and follow-up appointments scheduled.  The treatment plan will also be shared with your physician.

3. Your Therapy Program

How Many Therapy Sessions Will I Have?

Your therapy session frequency and duration will be based on the recommendations and plan established at your initial therapy evaluation. This can be adjusted during monthly progress reports.  Your visits will always be one to one with your therapist so every minute spent is about helping you reach your goals. Sessions are usually scheduled weekly and this is done directly with your therapist.

What Do I Wear?

Please dress comfortably for your therapy sessions. Your therapist will make suggestions if needed based on your therapy needs (such as aquatic therapy).

When Will I Be Discharged?

Discharge planning is done between you and your therapist. If needed at discharge, or at any time throughout your therapy session, we will help with referrals and information that you may need including community resources, DME equipment, mobility aids and more.  ​​​​
