Thank you for trusting us with your therapy care needs. We will do our utmost to uphold that trust and provide you with the highest quality of care and service. The following describes the steps we follow to start care and gives helpful information for our new or returning patients. Always reach out if you have questions.

Therapy Orders

If your therapy will be billed to your insurance, Medicare and most major insurance carriers require a physician or medical provider to oversee your therapy plan of care. You can request an order directly from your doctor, use our referral form or ask our Admissions Team to request on your behalf.

Admissions

With therapy orders, our Admissions Department will call you to complete your admissions paperwork and obtain insurance information. Once all information has been verified, we will match you with a therapist in your area with expertise in caring for your condition.   

Intake Paperwork

Well... you really can't have medical care without some sort of paperwork to complete. This can be done at the clinic if coming to our center for your therapy appointments or your therapist will bring to you to complete before the therapy evaluation. If you want to save a few minutes of your therapy time, you may also use the link below to complete and submit.

Your First Therapy Appointment

You may have one or more therapy disciplines recommended for your care. Each therapist will call you to schedule your initial therapy evaluation. You may choose to meet the therapist at our clinic or receive your therapy care at home. We encourage family and caregivers involved in your care to participate in the appointment if you wish.

For most patients, the initial therapy evaluation will last between 1 to 1 1/2 hours. During this time, your therapist will review your history with you, learn more about your concerns and goals for therapy and conduct a series of assessments to evaluate your condition and to create a customized treatment plan. The plan will be discussed with you before the end of the visit and follow-up appointments scheduled.  The treatment plan will also be shared with your physician.

Therapy Sessions

Your therapy session frequency and duration will be based on the recommendations and plan established at your initial therapy evaluation. Your visits will always be one to one with your therapist so every minute spent is about helping you reach your goals. Sessions are usually scheduled weekly and this is done directly with your therapist.

What to Wear

Please dress comfortably for your therapy sessions. Your therapist will make suggestions if needed based on your therapy needs (such as aquatic therapy).

Cancellations

Your therapist commits their time to you and cancellations disrupt progress and your therapist's ability to care for other patients. Please give as much notice as possible if you need to cancel a scheduled visit.   

Discharge

Discharge planning is done between you and your therapist. If needed at discharge, or at any time throughout your therapy session, we will help with referrals and information that you may need including community resources, DME equipment, mobility aids and more.
